Last year, we wrote about how junior doctors could be involved in Quality Improvement and about how the PDSA (Plan, Do, Study, Act) cycle is a better way of driving improvement that clinical audit. It is interesting that this systematic review of QI projects reports that only 15% of projects use the PDSA cycle properly and actually adhere to the notion of small scale change.
The fact that so few published projects do PDSA well highlights what I hear when I discuss QI with my colleagues: Doing a good quality QI project is actually quite difficult, often for a variety of reasons. QI is evidently an important undertaking, but it would be interesting to discuss some of the problems and also how some of you have overcome these difficulties.
So, the question is, what difficulties have you encountered when trying to carry out a QI project?
